Admission and Student Affairs
I. Personal Data
All applicants must read this part before providing any Personal Data to the Hospital.
- The Prince Philip Dental Hospital and The University of Hong Kong staff members may ask you to provide your Personal Data for purposes related to your prospective enrolment in the Hospital’s courses.
- When you provide your Personal Data to us, please make sure that the data are accurate and complete. If you fail to provide us with the information required or if the information provided is inaccurate or incomplete, your application for enrolment will be affected.
- Please also note that your Personal Data may be made available to:
- appropriate persons in the Hospital or The University of Hong Kong;
- Government or any other relevant parties who require it for matters related to your prospective enrolment; or
- any relevant Government departments/appropriate authorities when the Hospital is required to provide them under the relevant legislation for the purposes of that legislation.

In addition to the above, we will only use, disclose or transfer the Personal Data you provide to us:
- for those purposes relating to your prospective enrolment or other directly related purposes; or
- where permitted by law.
- We will obtain your consent before using your Personal Data for any other purposes.
- If you wish to access to or correct your Personal Data, please contact the Dental Ancillary Student Affairs Office during office hours [8:30 a.m. to 12:30 p.m. and 1:30 p.m. to 5:30 p.m. of every Monday to Friday (except public holidays)].
II. Application Method
1. General Diploma Course in Dental Technology and Certificate Course of Proficiency in Dental Surgery Assisting
Application Form for these two courses is attached in the middle of this Prospectus. You may submit the completed Application Form with payment of an application fee* to The Prince Philip Dental Hospital [Attention: Dental Ancillary Student Affairs Office] by mail or in person. DO NOT send cash by mail.

* You may apply for more than one course (with order of preference) on the same Application Form. The order of preference will affect the admission result. Please fill in the order with caution. The application fee is HK$150 per application, regardless of the number of courses being applied. Cheques should be made payable to The Prince Philip Dental Hospital. No refund of application fee will be made unless the course is cancelled. Should you wish to change the order after submission of the Application Form, you may complete an Amendment Form which is obtainable from the Hospital and return it to the Hospital as soon as possible but before the interview.
2. Higher Diploma Course in Dental Hygiene
(a) Application Form for this course is obtainable / downloadable from:
(i) HKU SPACE Enrolment Centres (please refer to the table below);
(ii) Information Counter of The Prince Philip Dental Hospital; or
(iii) HKU SPACE Community College Website (http://hkuspace.hku.hk/cc)
The completed Form should be returned to those HKU SPACE Enrolment Centres by mail or in person together with the application fee of HK$200 (crossed cheques or cashier order should be made payable to "HKU SPACE". DO NOT send cash by mail) as well as the following documents:
(i) photocopy of your HKID card
(ii) photocopies of academic transcripts, certificates and examination results
HKU SPACE also accepts on-line application at http://hkuspace.hku.hk/cc.
