Admission and Student Affairs入學申請及學生事務

  1. Personal Data

    All applicants must read this part before providing any Personal Data to the Hospital.

    個人資料

    所有申請人向本院提供任何個人資料之前，必須先閱讀本部份。

    1. The Prince Philip Dental Hospital ("Hospital") and The University of Hong Kong ("HKU") staff members may ask you to provide your Personal Data for purposes related to your prospective enrolment in the Hospital's courses.
      菲臘牙科醫院(下稱「本院」)或香港大學之職員可能會請你提供你的個人資料，作為有關入學申請之用途。
    2. When you provide your Personal Data to us, please make sure that the data are accurate and complete. If you fail to provide us with the information required or if the information provided is inaccurate or incomplete, your application for enrolment will be affected.
      當你提供自己的個人資料時，請確保該些資料準確及完整。如果你不能向本院提供所需的資料或你提供了錯誤或不完整的資料，你的入學申請將會受到影響。
    3. Please also note that your Personal Data may be made available to:
      你的個人資料可能會被交予：
      1. appropriate persons in the Hospital or HKU;
        本院或香港大學內有關人士；
      2. Government or any other relevant parties who require them for matters related to your prospective enrolment; or
        政府或其他有關人士，以便處理跟你的入學申請或相關事宜；或
      3. any relevant Government departments/appropriate authorities when the Hospital is required to provide them under the relevant legislation for the purposes of that legislation.
        任何有關的政府部門/管理機構，以符合有關法例的要求。
    4. In addition to the above, we shall only use, disclose or transfer the Personal Data you provide to us:
      除了上文所述以外，本院只會在下列情況下使用、透露或轉移你向本院提供的個人資料：
      1. for those purposes relating to your prospective enrolment or other directly related purposes; or
        作為入學申請或其他直接有關的目的；或
      2. where permitted by law.
        在法律容許的情況下。
    5. We will obtain your consent before using your Personal Data for any other purposes.
      在使用你的個人資料於其他用途前，本院會先向你取得同意。
    6. If you wish to access to or correct your Personal Data, please contact the Dental Ancillary Student Affairs Office during office hours [8:30 a.m. to 12:30 p.m. and 1:30 p.m. to 5:30 p.m. of every Monday to Friday (except public holidays)].
      如果你要求查閱或更改你的個人資料，請於辦公時間內與本院牙科輔助學生事務處聯絡【星期一至五上午8:30至下午12:30及下午1:30至5:30（公眾假期除外）】。
  2. Application Methods
    申請辦法
    1. General Diploma Course in Dental Technology
      and Certificate Course of Proficiency in Dental Surgery Assisting

      Application Form for these two courses is attached in this Prospectus. Applicant may submit the completed Application Form with payment of an application fee * to the Hospital [Attention: Dental Ancillary Student Affairs Office] by mail or in person. DO NOT send cash by mail.

      *Applicant may apply for more than one course (in order of preference) on the same Application Form. As the order of preference will affect the admission result, applicant is advised to fill in the order with caution. The application fee is HK$150 per application, regardless of the number of courses being applied for. Crossed cheque or cashier order should be made payable to The Prince Philip Dental Hospital. No refund of application fee will be made unless the course is cancelled. Should applicant wish to change the order after submission of the Application Form, he / she should complete an Amendment Form which is obtainable from the Hospital and downloadable from the Hospital's website (www.ppdh.org.hk), and return it to the Hospital before the interview.
      牙科工藝文憑課程及牙科手術助理員證書課程

      上述兩個課程的申請表格已刊於本概覽內頁。申請人可親臨菲臘牙科醫院或以郵寄方式［信封上請註明牙科輔助學生事務處收］遞交已填妥的申請表格及報名費*。切勿郵寄現金。

      *申請人可於同一份申請表格上，同時報讀兩個課程 (須列明選讀次序)。此次序將會影響取錄結果，請小心填寫。無論報讀課程數目多寡，每份申請只須繳付一次報名費，即港幣150元正。劃線支票或銀行本票抬頭請書「菲臘牙科醫院」。除課程被取消外，報名費一概不獲退還。如申請人在遞交申請表格後，希望更改課程的選讀次序，可向本院索取或往本院網頁(www.ppdh.org.hk)下載更改表格，填寫後於面試日前交回本院。
    2. Higher Diploma Course in Dental Hygiene
      牙科衞生護理高級文憑課程
      1. Application Form for this course is obtainable / downloadable from:
        本課程申請表格可於下列地點索取：
        1. HKU SPACE Enrolment Centres (please refer to the table below);
          香港大學專業進修學院各報名中心 (見下表)；
        2. Information Counter of The Prince Philip Dental Hospital; or
          菲臘牙科醫院一樓詢問處；或
        3. HKU SPACE Community College Website (www.hkuspace.hku.hk/cc)
          從香港大學專業進修學院網頁下載 (www.hkuspace.hku.hk/cc)

        The completed Form should be returned to those HKU SPACE Enrolment Centres by mail or in person together with the application fee of HK$200 as well as the following documents:

        申請表格必須連同報名費港幣200元正及下列證明文件一併交往香港大學專業進修學院各報名中心:

        1. photocopy of your HKID card
          香港身份證副本
        2. photocopies of academic transcripts, certificates and examination results
          學歷證明副本

        Crossed cheque or cashier order should be made payable to "HKU SPACE". DO NOT send cash by mail. HKU SPACE also accepts on-line application at www.hkuspace.hku.hk/cc.

        劃線支票或銀行本票抬頭請書「香港大學專業進修學院」，請勿郵寄現金。香港大學專業進修學院亦接受網上報名，申請人可登入學院網頁辦理申請手續（www.hkuspace.hku.hk/cc)。

      4. Other qualified applicants will be invited to attend admission interview.
        未能於特定招生日遞交申請表格的合格申請人，學院將另行通知面試地點及時間。
